• Healthy4Life Employee Health Care Center will be available to all City of St. Cloud employees and retirees and their dependents covered on the Medical Plan.
• Services provided to registered members will have no out-of-pocket costs.
• All visits to Healthy4Life Employee Health Care Center will be done by appointment, which means no long wait times. Walk-ins are accommodated based on schedule availability.
• Hours of Operation
  • Monday, Tuesday, Thursday, and Friday: 7:00 am to 3:30 pm closed between 11:30 am - 12:00 pm
  • Wednesday 10:30 am to 7:00 pm - closed 1:30 pm - 2:00 pm
  • Saturday and Sunday - CLOSED
• On-site laboratory services, available Monday through Friday
• Scope of services includes routine checkups, sick visits, and acute condition treatment.
• Chronic illness evaluation, treatment, and management are available.
